TNF inhibitors were the first blockbuster biologics and they still anchor treatment across inflammatory disease. Tumour necrosis factor is a signalling protein that drives inflammation; mopping it up calms rheumatoid arthritis, psoriasis, inflammatory bowel disease and ankylosing spondylitis. They differ mainly in structure and dosing — some are fully human antibodies, one is a fusion protein, one is a PEGylated fragment — which affects how often they are injected and how they behave in pregnancy.
