IL-23 inhibitors block interleukin-23, a cytokine that sustains the inflammatory T-cells behind psoriasis and inflammatory bowel disease. Targeting IL-23 specifically — rather than the broader immune suppression of older drugs — has produced some of the highest skin-clearance rates ever seen in psoriasis, with dosing as infrequent as every 12 weeks. Ustekinumab, the oldest of the group, blocks IL-12 as well as IL-23.
